    Dear Worldwide Woodturners,

    I am requesting copies of the following documents as required for public disclosure under IRS Section 6104:

    • IRS Determination Letter

    • Form 1023-EZ (Application for Tax-Exempt Status)

    • IRS Form 990 (Annual Information Return) for the past three years

    I understand that the organization may charge the IRS-permitted fees for copying and actual postage costs associated with this request. Please let me know the total amount due [including 3rd party payment processor fees] and the method of payment: requestor must provide a valid email address, telephone number, address; have a valid electronic payment method via CC and typical 3rd party processor account via PayPal, Venmo, Stripe, Zelle, et al; personal checks are not accepted.

    I also understand that requests will be processed within 30 days of receipt, which is within the IRS standard of 60 days.
